1. WHAT IS TELEHEALTH

Telehealth (also called telemedicine) is the delivery of healthcare services using electronic communications technology when the patient and healthcare provider are not in the same physical location. The telehealth services provided through GetMedKits may include telephone consultations (audio-only), asynchronous clinical review (a physician reviews your submitted health information and makes prescribing decisions without a live interaction), and electronic communications including text messages and emails related to your care.

You understand that telehealth involves the communication of your medical information using electronic technology.

2. HOW OUR TELEHEALTH SERVICES WORK

Our telehealth process involves two distinct steps:

Step 1: Administrative Intake Call. You will speak with a patient coordinator by telephone. This is a non-clinical informational call. The patient coordinator is not a physician, nurse practitioner, or licensed healthcare provider. This call does not constitute a medical consultation, does not establish a doctor-patient relationship, and no clinical decisions are made during this call. The purpose is to explain how the service works, answer your logistical questions, and assist with registration.

Step 2: Independent Physician Review. After you submit your medical intake form, an independently licensed physician or nurse practitioner will review your health information, medical history, current medications, and treatment goals. The physician will determine, based on their independent clinical judgment, whether prescribing medication is appropriate for your individual situation. The physician may contact you directly if additional information is needed. The physician may approve, modify, or decline your request entirely.

You understand and agree that the physician's review and prescribing decision is made independently of the initial informational call. The physician exercises sole clinical judgment over all medical decisions.

3. BENEFITS OF TELEHEALTH

Telehealth may offer the following potential benefits: improved access to healthcare services regardless of your geographic location within the United States, convenience of receiving care without traveling to a physical office, reduced exposure to illness in clinical waiting areas, timely access to physician review and prescription services, and the ability to consult with specialists who may not be available in your local area.

4. LIMITATIONS OF TELEHEALTH

You understand and acknowledge the following limitations:

The physician cannot perform a physical examination of you through telehealth. This means that certain conditions, symptoms, or findings that might be detected during an in-person physical examination may not be identified through telehealth.

Telehealth services may not be suitable for all medical conditions. The treating physician may determine that your condition requires an in-person evaluation and may decline to provide telehealth services or may refer you to an in-person provider.

The quality and availability of telehealth services depend on your telephone connection and the accuracy of the information you provide. Poor connection quality, incomplete information, or technical difficulties may affect the quality of your care.

Telehealth does not replace your relationship with your primary care physician, oncologist, or other treating specialists. You are strongly encouraged to maintain all existing provider relationships.

The information you provide during the intake process and on your intake form serves as the primary basis for the physician's clinical assessment. The physician's ability to evaluate your condition is limited to the information you provide. Incomplete or inaccurate information may result in inappropriate treatment recommendations.

5. RISKS OF TELEHEALTH

You understand that telehealth carries the following risks:

Information transmitted electronically may be insufficient for accurate diagnosis or appropriate treatment in some cases.

Delays in evaluation or treatment may occur due to technology failures, connectivity issues, or other technical problems.

Security measures and encryption are used to protect the privacy and confidentiality of your health information, but no technology is completely secure. There is a potential risk that your health information could be intercepted or accessed by unauthorized parties despite these safeguards.

In rare cases, information transmitted may be lost due to technical failures. Neither GetMedKits nor the treating physician shall be held liable for information lost due to technical failures of electronic communications systems.

Standard telephone communications (audio-only calls) are not encrypted and may carry a risk of interception.

The physician's assessment is based on the information available through the telehealth encounter and may differ from a diagnosis or treatment recommendation that could result from an in-person examination.

Despite reasonable efforts, the telehealth encounter may not result in a prescription being issued. The physician may determine that telehealth services are not appropriate for your condition.

6. YOUR RIGHT TO REFUSE TELEHEALTH

You have the right to refuse telehealth services at any time without affecting your right to future care or treatment. You have the right to withdraw your consent to telehealth at any time. You have the right to request an in-person evaluation instead of telehealth services. Your decision to refuse or withdraw from telehealth services will not result in any penalty or loss of benefits to which you are otherwise entitled.

If you choose to refuse telehealth services, we will assist you in identifying in-person healthcare options in your area when possible.

7. OFF-LABEL PRESCRIBING

The medications available through this service, including ivermectin, mebendazole, and methylene blue, may be prescribed for off-label uses. This means the medications are FDA-approved for specific indications, but the physician may prescribe them for uses that the FDA has not specifically approved.

Off-label prescribing is a legal and common practice in medicine. The FDA recognizes that once a drug is approved, healthcare providers may prescribe it for uses other than those studied and approved when they judge it to be medically appropriate.

None of the medications available through this service are FDA-approved for the treatment, cure, prevention, or mitigation of cancer, any oncologic condition, COVID-19, or any viral illness.

The scientific evidence supporting the off-label uses of these medications varies in quality and quantity. The potential anticancer properties of these medications are supported by preclinical research and limited clinical observations. No large-scale randomized controlled trials have established their efficacy as cancer therapies in humans.

No guarantee of efficacy or specific health outcome is made or implied. You understand that these medications may or may not be effective for your condition.

You acknowledge that you have been advised to discuss the current state of the evidence, the limitations of that evidence, and all alternative treatment options with your prescribing physician and your oncologist before beginning any protocol.

8. COMPOUNDED MEDICATIONS

The medications dispensed through this service are compounded by a licensed U.S. compounding pharmacy operating under Section 503A of the Federal Food, Drug, and Cosmetic Act. Compounded medications are individually prepared by a licensed pharmacist in response to a valid, patient-specific prescription.

COMPOUNDED MEDICATIONS ARE NOT FDA-APPROVED. The FDA does not verify the safety, effectiveness, or quality of compounded medications before they are dispensed. Compounded medications may carry risks beyond those of commercially manufactured drugs, including variability in potency, purity, stability, and sterility.

The compounding pharmacy operates independently from GetMedKits and is solely responsible for the quality of the medications it prepares.

You acknowledge that you understand these risks and accept them as part of your decision to use compounded medications prescribed through this service.

9. RISKS AND SIDE EFFECTS OF SPECIFIC MEDICATIONS

All medications carry risks of side effects, adverse reactions, and drug interactions. The following is a summary of known risks for the medications available through this service. This is not a complete list. Your prescribing physician and the dispensing pharmacy will provide additional information specific to your prescription.

Ivermectin: Side effects may include nausea, dizziness, skin rash, diarrhea, and in rare cases neurological effects at high doses.

Mebendazole: Side effects may include abdominal pain, diarrhea, flatulence, and in rare cases elevated liver enzymes with prolonged use.

Methylene blue: Side effects may include discoloration of urine and stool (blue-green color, this is expected and harmless), nausea, headache, and dizziness. Methylene blue may interact with serotonergic medications including SSRIs (such as fluoxetine, sertraline, escitalopram), SNRIs (such as venlafaxine, duloxetine), MAOIs, and other serotonin-modulating drugs. This interaction can potentially cause serotonin syndrome, a serious and potentially life-threatening condition requiring immediate emergency medical attention.

You must inform your prescribing physician of ALL medications, supplements, over-the-counter drugs, herbal products, and treatments you are currently taking or have recently taken. Failure to disclose this information may result in serious adverse drug interactions.

If you experience any adverse reaction, stop taking the medication immediately and seek emergency medical attention. Call 911 or contact Poison Control at 1-800-222-1222.

10. ADJUNCTIVE USE ONLY

The protocols offered through this service are intended to be used alongside your existing cancer treatment, not as a replacement for it. You are strongly encouraged to maintain your relationship with your oncologist and primary care physician. Do not stop, change, or delay any conventional cancer treatment based on information from this service without first consulting your oncologist.

You must inform both your oncologist and your prescribing physician through this service about all treatments and medications you are currently receiving.

11. YOUR RESPONSIBILITIES

You agree to the following responsibilities as a telehealth patient:

Provide truthful, accurate, and complete information about your identity, health history, current medications, supplements, allergies, and current treatments on all forms and during all communications.

Promptly inform your treating physician and GetMedKits of any changes in your health status, medications, or treatments.

Follow the instructions and dosing guidelines provided by your prescribing physician and the dispensing pharmacy.

Read all medication information, labels, patient information sheets, and medication guides provided with your prescription.

Use prescribed medications for your personal use only. Do not share, sell, or distribute medications to any other person.

Maintain a suitable environment for telephone consultations, free from distractions and where you can speak privately about your health.

Call 911 or go to the nearest emergency room if you experience a medical emergency. Do not rely on this service for emergency care.

12. PRIVACY AND CONFIDENTIALITY

Your health information is protected by applicable federal and state privacy laws. We use administrative, technical, and physical safeguards to protect your information. All electronic health records created during or as a result of your telehealth encounter are maintained in accordance with applicable law.

Your health information will be shared with your treating physician for clinical review and prescribing, and with the compounding pharmacy for prescription fulfillment. Your information will not be shared with any third party for purposes unrelated to your care without your consent, except as required by law.

Telephone calls may be recorded for quality assurance, training, treatment documentation, and compliance purposes. By participating in a telephone call, you consent to recording. If you are in a state that requires all-party consent for call recording (California, Connecticut, Delaware, Florida, Illinois, Maryland, Massachusetts, Montana, Nevada, New Hampshire, Pennsylvania, or Washington), you will hear a verbal disclosure at the beginning of the call. By continuing the call, you consent to recording.

13. NO EMERGENCY SERVICES

14. MEDICAL RECORDS

Medical records generated through your telehealth encounter, including your intake form, physician review notes, and prescription records, will be maintained in accordance with applicable federal and state medical records retention laws. Your consent to receive telehealth services and the information you provide during the telehealth process will be documented in your medical record.

15. PROVIDER INFORMATION

The healthcare providers who review your case and make prescribing decisions are independently licensed physicians and nurse practitioners. They are licensed to practice medicine in the state where you are located at the time of service. They are not employees of GetMedKits. They exercise independent clinical judgment over all medical decisions.

You have the right to request the name, credentials, and licensing information of your treating provider at any time by contacting us.

16. FEES AND PAYMENT

This is a cash-pay service. We do not accept insurance, Medicare, Medicaid, or any other third-party payor. All fees are communicated to you verbally before payment is collected. Payment is processed over the phone, not through the website. You agree that you will not submit claims to any insurer or government program for reimbursement.

17. NO CONTROLLED SUBSTANCES

This service does not prescribe controlled substances as defined by the DEA under Schedules I through V of the Controlled Substances Act. If your condition requires controlled substances, please consult your primary care physician or specialist directly.

18. STATE-SPECIFIC DISCLOSURES

The following disclosures are required by the laws of specific states. If you are a resident of any of the following states, the corresponding disclosure applies to you:

Texas: The Texas Medical Board investigates complaints relating to the practice of medicine. To file a complaint, call 1-800-201-9353 or visit the Texas Medical Board website at tmb.state.tx.us. You may also write to the Texas Medical Board at P.O. Box 2018, Austin, TX 78768-2018. A brochure explaining the complaint process is available upon request.

California: You have the right to file a complaint with the Medical Board of California by calling 1-800-633-2322 or visiting mbc.ca.gov.

Nebraska: Before receiving telehealth services, you have the right to be informed of the following: (a) you have the right to refuse telehealth services at any time without affecting your right to future care or treatment; (b) all existing confidentiality protections apply to information obtained or disclosed during a telehealth consultation; (c) you have the right to access all medical information resulting from the telehealth consultation as provided by applicable law; and (d) your identifiable health information will not be disseminated to any third party without your written consent, except as required by law. By providing your consent to this Telehealth Consent, you acknowledge that you have been informed of these rights.

Oklahoma: You have the right to withhold or withdraw consent for telehealth services at any time without affecting your right to future care or treatment.

Utah: You are entitled to the following information regarding telehealth services: (a) any additional fees for telehealth and methods of payment; (b) the persons to whom your identifiable health information may be disclosed, and the purposes of such disclosure; (c) your rights regarding your health information; (d) appropriate uses and limitations of telehealth; (e) security standards applied to protect your health information under HIPAA and HITECH; (f) potential risks to your privacy despite these security measures; (g) the risk that information may be lost due to technical failure; and (h) identification of the parties operating the telehealth platform.

Colorado: (a) You may refuse telehealth services without loss of any right to future care or treatment and without risking the loss or withdrawal of any benefits; (b) all existing confidentiality protections apply to telehealth services; and (c) you have access to all medical information resulting from telehealth services as provided by applicable law.

Kentucky: You understand that telehealth has limitations, including the inability to perform a physical examination, which may affect the accuracy of diagnosis or treatment. You understand that there are potential risks to the confidentiality of your health information due to the use of electronic communication technology.

Maryland: Your treating provider will disclose the following before providing telehealth services: the provider's name, contact information, and license type; potential risks and benefits of telehealth; the circumstances under which the provider may determine that telehealth is not appropriate; your right to refuse or withdraw from telehealth at any time; emergency safety protocols; and policies for maintaining your medical records.

If the physician approves a prescription, it is sent to a licensed U.S. compounding pharmacy, where your medication is individually prepared and shipped to you.

IMPORTANT RISKS AND SIDE EFFECTS
